The Jordanian community has been facing restricted access to healthcare. The financial constraints, lack of insurance, and limited access to primary care physicians contribute to worsening health outcomes and increased complications from treatable conditions (Capella University, n.d.). Jordan’s healthcare system lacks sufficient primary care physicians, which has caused community members to visit urgent care clinics to treat their unaddressed chronic diseases (Capella University, n.d.). Moreover, According to Vila Health (n.d.), there is one physician for every 1111 insured residents in Jordan.

Consequently, this ratio overlooks a considerable portion of the population, indicating that many individuals do not have access to medical care.  In Scot County, including Jordan, about four percent of dwellers lack health insurance. Furthermore, within Scott County, encompassing Jordan, a ratio of 915 residents to one mental health provider exists, whereas the statewide ratio for Minnesota is 430 residents per mental health provider (St. Francis Regional Medical Center, 2020).  This evident lack of healthcare access for chronic disease management in Jordan necessitates a strategic change to enhance the community’s accessibility to essential healthcare services.  These issues will be resolved by improving access to healthcare services in the Jordan community.

The proposed changes to the healthcare system, mainly focusing on improving access to healthcare for chronic disease management, offer several direct benefits to the community of Jordan. Access to healthcare services and chronic disease management can improve community members’ health outcomes. Furthermore, it will reduce the burden of untreated conditions and prevent complications that often result in emergency room visits or hospitalizations (McMaughan et al., 2020). Similarly, by providing enhanced access to healthcare services for the older population of Jordan will lead to better management of their chronic diseases. Ultimately, their health outcomes will be improved, leading to enhanced quality of life. 

The improvements in overall health resulting from these changes have significant implications for the community of Jordan. With improved access to healthcare, reduced emergency room visits for preventable conditions, and a general improvement in the community’s health status. Moreover, there will be a substantial reduction in mortality rates and overall better management of chronic health conditions (May et al., 2021).  These positive outcomes can contribute to a more resilient and thriving community, fostering economic growth and social cohesion.

Potential Barriers to Change

 Improving access to healthcare for chronic disease management in the Jordanian community may present several potential barriers to change. Participants in Jordan community discussions and individuals with insider knowledge highlighted that difficulties with transportation, lack of healthcare trust, and the absence of health insurance were obstacles to accessing healthcare (St. Francis Regional Medical Center, 2020). According to the Transtheoretical Model (TTM), individuals go through stages of change: pre-contemplation, contemplation, preparation, action, and maintenance (Del Rio Szupszynski & de Ávila, 2021). Some individuals may not perceive the need for change or may lack motivation due to factors such as fatalism, traditional beliefs, or socioeconomic constraints (Del Rio Szupszynski & de Ávila, 2021).

Additionally, health delivery by web-based platforms for elderly patients can be troublesome and irrelevant due to a lack of knowledge of their usability (St. Francis Regional Medical Center, 2020). Furthermore, implementing systemic changes requires collaboration across multiple sectors, such as Scott County Mobile Medical Center St. Mary’s Health clinics (St. Francis Regional Medical Center, 2020). However, navigating complex bureaucratic processes, overcoming resistance from entrenched interests, and fostering collaboration among diverse stakeholders requires time, effort, and leadership (Airhihenbuwa et al., 2021). 

Strategies for Changing Barriers into Opportunities and Resolving Conflict

The healthcare system in Jordan, as highlighted by Capella University (n.d.), is grappling with significant challenges, including a lack of healthcare insurance and limited access to primary care physicians for managing healthcare conditions, particularly chronic diseases. To address these obstacles, a multifaceted approach, as identified by participants in Jordan community discussions and individuals with insider knowledge, is imperative. Vila Health executives, renowned for their expertise and innovative solutions, can play a pivotal role in this process.

Firstly, in collaboration with local stakeholders, Vila Health can support initiatives to mitigate transportation challenges by leveraging their resources and expertise to establish community transportation services or forge partnerships with local providers. Additionally, Vila Health can contribute to integrating telehealth options, as suggested by Turner & Upton (2021), to reduce the need for physical travel and enhance access to healthcare services, especially in underserved areas. Furthermore, Vila Health executives can lend their credibility and experience to support community outreach programs to build trust in healthcare and foster transparent communication between providers and residents. They can also advocate for policy changes to increase health insurance coverage and assist in implementing assistance programs to facilitate enrollment, thereby addressing barriers to access.

Resolving conflicts during implementation requires transparent communication, active listening, a commitment to finding mutually acceptable solutions, and the implementation of proactive conflict resolution strategies. This includes establishing clear channels for conflict resolution, such as designated mediators or advisory committees, to address differences of opinion and conflicting interests promptly. Moreover, promoting a culture of collaboration and cooperation among stakeholders encourages respectful dialogue and problem-solving techniques, ensuring that conflicts are managed constructively (Konrad et al., 2020). By adopting these comprehensive strategies, Jordan can create a supportive environment conducive to improving healthcare access, ultimately leading to better outcomes for individuals and the community.

It is imperative to collaborate and communicate with relevant stakeholders, including local school managers, hospital administrators, healthcare providers, local funding agencies, and leadership personnel, to implement the changes proposed for the Jordanian community effectively. One effective strategy for helping organizational stakeholders comprehend and estimate proposed changes is to organize informative sessions tailored to their specific information needs (Turner & Upton, 2021). These sessions can serve as a platform for presenting the rationale behind the proposed changes, outlining their potential impact on the organization, and addressing any concerns or questions stakeholders may have. 

The proposed changes are likely to affect the organization in several ways. Improving access to healthcare for chronic disease management may involve changes to scheduling, resource allocation, and patient communication strategies. These changes may impact workflows, staff responsibilities, patient outcomes, and overall organizational performance (McMaughan et al., 2020).

To support the proposal, Vila Health executives will need data and information on several key areas, including: 

  • Data on barriers to healthcare access, such as patient wait times, staff shortages, or patient satisfaction scores.
  • Research and case studies demonstrate the proposed interventions’ effectiveness, including outcomes data from similar organizations or initiatives.
  • Detailed financial projections, including estimates of implementation costs, ongoing expenses, and potential return on investment. 

With this strategy and adequate data and information supporting proposed changes, stakeholder collaboration can improve Jordan’s health condition by increasing healthcare access and for better chronic disease management.

Considering the size of Jordan as a small town, even a relatively small percentage of the population (7000 people) experiencing these challenges can have a notable impact on the community (City of Jordan, 2015). 

Several barriers are likely to be encountered when implementing this change in Jordan. These barriers may include transportation issues, lack of trust and poor knowledge of telehealth use. The sources of information used to define the need for change include the Vila Health Jordan Community Assessment, which provides insights into the healthcare needs and challenges faced by the community. Additionally, data on the Jordan community, such as demographics, health indicators, and utilization of healthcare services, can inform the need for targeted interventions to address chronic disease management. Other change initiatives or studies addressing similar problems include efforts by local organizations, healthcare providers, and community groups to provide resources for chronic disease management, and improve access to healthcare services. Collaborating with these initiatives and leveraging existing resources and expertise can enhance the effectiveness and sustainability of the proposed change in Jordan.

Program Description

The proposed change initiative aims to improve access to healthcare for chronic disease management in Jordan. Therefore, efforts will be made to improve access to healthcare services for chronic disease management through expanding telehealth services, increasing outreach to underserved populations,  increasing health insurance rates among community members, and enhancing coordination between healthcare providers and community resources. The initiative will be implemented over a specified timeline, with phases and activities planned for gradual rollout. Timelines may vary depending on the specific components of the initiative and resource availability. 

Implementation will occur across various settings within the community of Jordan, including  healthcare facilities, community centers, and local outreach programs, ensuring accessibility and reach to all community members. The initiative is necessary to address the pressing need for improved healthcare access in Jordan. Chronic diseases affect a significant portion of the population, and the current lack of resources and awareness exacerbates these challenges (Capella University, n.d.). The initiative aims to promote overall well-being, reduce healthcare disparities, and enhance the quality of life for individuals and families in the community by implementing comprehensive programs and improving access to care.

Implementation will involve forming interdisciplinary teams to oversee program development and implementation, conducting needs assessments to identify specific areas of focus, securing funding and resources, establishing partnerships with local organizations and stakeholders, developing educational materials and resources, training educators and healthcare providers, and evaluating the effectiveness of the programs through ongoing monitoring and feedback mechanisms. Additionally, community engagement and outreach efforts will be integral to ensuring the success and sustainability of the initiative.

Goals and Objectives

The proposed change initiative aims to improve chronic disease management. These objectives involve expanding telehealth services, establishing support networks, and enhancing stakeholder collaboration. Evaluation will occur periodically, with specific milestones established for assessment. This systematic approach ensures accountability, transparency, and continuous improvement in achieving the initiative’s goals.
